Player Information Form

Please provide the following information that iGaming Ontario (iGO) will use to verify your player account information.

iGaming Ontario is collecting your personal information under the Freedom of Information and Protection of Privacy Act (Ontario) (“FIPPA”), to process your withdrawal request subject to appropriate authorizations and verifications We ask you to provide identity verification information confirming that you are the verified holder of the player account.  We will pay you the owned amount using iGO’s approved payment methods. By providing your personal information, you certify that the information is accurate and acknowledge that by providing your email address, you agree to receive email communications regarding a final withdrawal of funds from your player account. We will only use your personal information for the purpose for which it is collected in accordance with applicable privacy laws.  Please contact igoprivacy@igamingontario.ca should you have specific privacy related questions.
